Office Policies: No Show Appointments: A $50.00 fee will be billed directly to the patient for all “no show” appointments Late Cancellations: A $50.00 fee will be billed directly to the patient for appointments cancelled without one full business days notice. Phone Consultations: Phone consultation fees usually vary between $70 and $175. Returned Checks: There will be a $25.00 processing fee for all returned checks. This includes checks returned due to NSF or stopped payment. Late Charges: A 1.5% late fee will be applied for patient balances greater than 60 days overdue. Prescription Policies: Children from all over North Carolina come to Clinic for Special Children to be treated by Dr. Harum, therefore written prescriptions that need mailing will require a one week notice to renew. All others may take up to 48 hours to renew. If you need a prescription phoned in to your pharmacy please leave the name and phone number of the pharmacy you wish us to call. It is helpful for our staff to know whether you want the prescription mailed to you or you will be picking it up here at the office. Insurance Information: With the exception of Medicaid and North Carolina Health Choice, we do not file insurance claims for our patients. However, we will provide you with all the necessary information to allow you to file for reimbursement from your health insurance carrier and are happy to advise you regarding the insurance filing process as requested. Laboratory Studies Information: Clinic for Special Children utilizes the services of several different labs in North Carolina. We are not financially affiliated with them in any way therefore you will be receiving their bill separately. If you live outside the Clinic area you may opt to have labs done at a facility closer to your home, in that case we can provide you with a written order from Dr. Harum. Low Risk Vaccine Program Policies:

Because we do not participate in North Carolina’s Vaccine for Children Program, which provides vaccines to patients at no cost, insurance companies will not always reimburse for the lower risk vaccines. However, CSC can bill your insurance company for the office visit, so if your child is receiving their immunizations at Clinic for Special Children you will need to be prepared to pay your deductible (if any), co-pay and the cost of the vaccine at the time of service. This Low Risk Vaccine Program is not limited to patients of Clinic for Special Children but is available to anyone. |
